Managing Forms

Stats cards

At the top of the page, four summary cards show aggregate metrics:

Filters

FilterDescription

Form cards

Forms are displayed in a two-column card grid with infinite scroll (more forms load as you scroll down).

Each form card shows

FieldDescription
"New" badgeAnimated orange badge — appears when the form has received a lead today
Status badgeACTIVE, ARCHIVED, or DRAFT
Campaign link badge"Linked" (with campaign name) or "Not linked"
Form nameName of the lead form
Last lead dateWhen the most recent lead was received
Questions countNumber of questions in the form (hover to see all questions in a tooltip)

KPI row

MetricDescription
Total leadsTotal number of leads for this form
Today's leadsLeads received today
7-day sparklineA mini bar chart showing daily lead volume for the past 7 days. Past days are purple, today is orange.

Campaign hierarchy

Below the KPIs, the form card shows the campaign hierarchy it belongs to:

Campaign Name
└── Ad Set Name
└── Ad Name
  • Created date — When the form was created on Facebook
  • Last synced — When the form's leads were last synced
  • View Leads button — Opens the Form Detail page

Syncing forms

Click the Sync Forms button to fetch the latest forms and lead submissions from Facebook.


Form detail

Click View Leads on any form card to open the form detail page.

  • Back button — Return to the form list
  • Form name and page ID
  • Total leads count
  • Export CSV button — Downloads all leads as a CSV file
  • Sync Leads button — Syncs the latest leads for this form

Questions bar

All form questions are displayed as chip pills (indigo colored) across the top of the page, showing the fields captured by this form.

Filters

Leads table

A dynamic table where columns are automatically generated based on the form's questions:

[Question fields]One column for each custom question in the form (e.g., city, budget, interests)
SourceHow the lead was received — "webhook" or "api" badge
Submitted AtWhen the lead submitted the form

Pagination: Server-side pagination with options for 10, 20, 50, or 100 leads per page.

Group By mode

Select a field from the Group By dropdown to switch to a summary view:

  • An expandable summary table replaces the normal leads table
  • Each row shows a group value, count, and percentage share (with a visual bar)
  • Click a row to expand it and see the individual leads in that group
  • The grouped column is hidden from the expanded sub-table to avoid duplication

1. On the form detail page, select a field from the Group By dropdown
2. Expand one of the group rows to show the nested leads sub-table

This is useful for analyzing lead distribution — for example, grouping by city to see which cities generate the most leads, or grouping by a custom question field to understand lead preferences.

Exporting leads

  1. Open a form's detail page
  2. Click the Export CSV button
  3. A CSV file is downloaded with all leads and their form fields

The CSV file is named leads-<formName>.csv and includes all captured fields and submission timestamps.

Tips

  • Watch for the "New" badge — Forms with leads today have an animated orange badge, making them easy to spot
  • Use Group By to quickly analyze lead segments without exporting
  • Export regularly — Facebook retains lead data for 90 days, so download your leads before they expire
  • Check the 7-day sparkline to spot trends — a declining bar chart may mean your ad is losing effectiveness
